Merrickville District Community Health Centre, Smiths Falls District Community Health Centre and Rideau Valley Diabetes Services together form the Merrickville District Community Health and Services Centre. All three components provide care for individuals and families and respond to community concerns.

Merrickville District Community Health and Services Centre was incorporated in 1974 as a non profit corporation.  It is owned by the community and governed by a Board of Directors of elected community volunteers.

Merrickville District Community Health and Services Centre offers a variety of primary health care, health education and health promotion services using a team approach.  The teams work with individuals, families and the community to promote health and prevent injury and illness.

MDCHSC is supported by the South East Local Health Integration Network (SE LHIN), The Ministry of Health and Long Term Care, the Ministry of Community and Social Services and the people and communities we serve.
