
Our two CHCs offer a wide variety of programs and services designed to meet the needs of the communities we serve. Our programs and services are provided by a variety of health professionals who work together to provide access in response to client and community needs.
The Primary Health Care Team consists of:
Working together, our team looks at the factors in our clients lives that impact their health and well-being: income, housing, social supports, employment, neighbourhood and access to healthy, affordable foods.
Our primary care clinic offers a highly skilled interdisciplinary team of family physicians, nurse practitioners and nurses who provide medical services to registered clients. The clinic team collaborates with CHC social workers, dietitians and health promoters to provide comprehensive client care and to address the wide range of factors that affect optimal health.
Foot care is available for those who are at high risk for foot complications from diseases such as diabetes, peripheral vascular disease, arthritis, poor vision, or inability to care for their own feet. Our Foot Care Nurses see clients once a week on Thursdays.
In partnership with the Tri-County Dental Coalition, a dentist provides services twice a month to clients with developmental and intellectual challenges. Contact Jean at 613-283-1952, ext 127.
Very limited spaces are also available through the Pro Bono Clinic for eligible residents of Leeds, Grenville and Lanark Counties. Craving Change educational and support group that help participants understand and improve their relationship with food. It offers appealing and practical strategies that help individuals improve their current eating habits. Contact Tanya: 613-283-1952, ext 125.
From October through March, walkers meet twice a week at the Smiths Falls Collegiate Institute to walk in the halls. Contact Mona at 613-283-1952, ext 136.
In partnership with the Smiths Falls Lions Club, the Good Food Box operates as a wholesale buying club that makes it more affordable to buy vegetables and fruit. Participants can place an order at the CHC for one of four sizes and types of fruit/vegetable boxes by the second Tuesday of the month. Pick up is on the third Tuesday of the month. One a month, men from the community get together to plan, prepare and nutrition meal while building their food knowledge and cooking skills. Contact Tanya: 613-283-1952, ext 125.
Healthy Living by the Rideau
In partnership with the Rideau Roundtable and Arts for Health and Learning, day programs are offered to groups of youth aged 11 and up that include canoeing or hiking, healthy snacks and lunch, and a creative arts project. Contact Mona 613-283-1952, ext 136.
A group weight management approach that focuses on lifestyle changes to help participants reach and maintain a healthier body weight. Participants will learn how nutrition, physical activity, self-esteem, their environment and specific behaviours affect weight. Contact Tanya: 613-283-1952, ext 125.
Our heart health workshop provides participants with tools to lower cholesterol, triglycerides and blood pressure through lifestyle changes and teaches cooking techniques using heart healthy recipes to lower fat, cholesterol and sugar in recipes and maintain flavour and texture. Living well focuses on self-management techniques for coping with a chronic disease such as, fibromyalgia, heart disease and many others. Topics covered include eating well, being active, coping with stress and getting support. Contact Jill at 613-283-1952, ext 134.

Our dietitian offers individual counselling to community residents who are concerned about managing their weight, lowering their blood pressure, lowering cholesterol, coping with an eating disorder and general questions and concerns relating to food. Contact Tanya: 613-283-1952, ext 125.

Merrickville and District CHC and Smiths Falls CHC staff contribute to a number of community partnerships and coalitions that contribute to the health and well-being of the residents of Merrickville, Smiths Falls and surrounding areas, including:
