Integration at WorkMDCHC works with clients and partners to develop health promotion activities and strategies to support a healthy community.  We look forward to continuing our relationships with the following organizations as we work together in primary care reform:

The Arthritis Society: Provides education and resources for individuals learning to manage their arthritis.

Tri-Health Coalition of Leeds, Grenville and Lanark: Tri-Health is a coalition of agencies and community partners promoting and encouraging healthy living with the residents of Leeds, Grenville and Lanark Counties. The vision of the Tri-Health Coalition is a smoke-free community that is physically active & makes healthy food choices in order to reduce the risk of cardiovascular disease and other chronic diseases.

Every Kid in Our Communities (EKIOC)-Leeds and Grenville: EKIOC is a coalition of agencies working together to support families and communities to build assets and promote the safe and healthy development of children and youth in Leeds and Grenville.

Smiths Falls FOCUS Coalition – A coalition of agencies and community partners working towards preventing alcohol and other drug misuse, associated injuries and chronic diseases.

Rideau Valley Diabetes Services - Provides accessible programming to residents of Lanark, Leeds and Grenville counties whose lives are affected by diabetes. RVDS is community-based and dedicated to assisting people in developing their strengths to live healthy, active lives and to building support for coordinated diabetes prevention and management.

Canadian Hearing Society (Brockville Branch) - The Canadian Hearing Society offers counselling, education, advocacy, information and referral services to assist individuals to manage and cope with their hearing loss. These services are also provided to family members, caregivers and service providers. In addition, staff demonstrates and recommends communication devices that will enhance a person’s ability to hear. All these services can be accessed by meeting with a counsellor at a Hearing Health Care Clinic.  Regular Hearing Health Care Clinics are offered at MDCHC.
